Lenox Care helps clients with both intensive and basic care services. Chapter 245D of the Minnesota Statutes regulates the provision of community and home-based services to people living with disabilities and those above 65 years.
245D services provided by Lenox Care are crucial in helping our clients regain independence in the community and in their homes. Our team of qualified personnel is dedicated to helping clients achieve their objectives and goals. First, we listen to our clients and what matters in their lives and then create the best strategy to get them there. Clients will stay connected to the community with the power to accomplish essential tasks in life. A sense of comfort and peace is guaranteed by having our professionals in your life.
Lenox Care offers Housing Stabilization Services. We strive to ensure our clients have access to housing, housing resources, and education to sustain the housing options they desire. We provide both Housing Transition and Housing Sustaining as well as Housing Consultation. Transition services are provided to those that need help plan for, find, and move into housing. Sustaining services are provided to those that need help maintain living in their current housing.
Basic support services
Individual community living supports (ILCS)
For people who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes.
24-hour emergency assistance
On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency.
Companion services
Non-medical care, supervision and socialization to a person age 18 or older.
Homemaker services
Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.
Night supervision
Provides overnight assistance and monitoring by an awake staff in the person's home.
Personal support
Services provided in the person's home or community in support of achieving potential, increasing independence and meeting community inclusion goals.
Respite care services
Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.
Intensive support services
Specialist services
Services designed to promote staff and caregiver competency to meet a person’s needs in eligible areas.
Crisis respite
Short-term care and intervention strategies provided to a person due to the need for caregiver relief, protection of the person or others living with the person or the person’s need for behavioral or medical intervention.
Independent living skills training
Services that develop, maintain and improve the community-living skills of a person.
Semi-independent living skills
Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community.
Day training and habilitation
Services that develop and maintain life skills for people with developmental disabilities or related conditions so they can fully participate in community life.
Structured day
Service designed for people who may benefit from continued rehabilitation and community integration directed at the development and maintenance of community living skills.
Housing Stabilization Services
Transition services
Services that assist a person to plan for, find, and move to a home in the community.
Sustaining Services
Services that supports a person to maintain living in their home in the community.
Consultation Services
People who do not have waiver or targeted case management or a MSHO/MSC+ care coordinator, but need Housing Stabilization Services, can complete the person-centered planning needed through an enrolled housing consultant.
Employment services
Prevocational services
These services will prepare people with disabilities for various jobs with competitive pay and assist them in getting greater independence in the community.
Employment development services
These services will assist people in securing integrated and competitive employment, in addition to becoming self-employed.
Employment exploration services
Such services assist people in becoming better aware of various integrated and competitive job opportunities in the community. In addition to an equitable approach, Lenox Care is committed to ensuring equal opportunities for employment and advancement for everyone.
Employment support services
Individualized services and supports that help people maintain paid employment in community businesses/settings. Employment support services occur in integrated community settings.
Supported employment
Effective July 1, 2018, supported employment services began the transition to the new disability waiver employment services.
